PI-RADS Report Assistant

Prostate MRI scoring helpers — image quality (PI-QUAL v2), gland volume + PSA-D, lesion-level PI-RADS v2.1. Stateless; nothing is saved.

Image Quality (PI-QUAL v2)

de Rooij 2024
Protocol:
T2-WI 0 / 4
T2-weighted imaging
Essential prerequisite
Slice thickness 3 mm
If unchecked, score is automatically 0/4.
1.Axial — adequate SNR in all parts of the image
2.Axial — clearly delineate relevant prostatic structures (capsule, seminal vesicles, ejaculatory ducts, NVB, ext. urethral sphincter)
3.Axial — absence of significant artifacts in the prostatic region
4a.Sagittal — adequate SNR & resolution AND absence of significant artifacts
4b.Coronal — adequate SNR & resolution AND absence of significant artifacts
Criterion 4: both sagittal and coronal must be adequate — if either is poor, the point is lost.
DWI 0 / 4
Diffusion-weighted imaging
Essential prerequisites
Slice thickness ≤ 4 mm
High b-value sequence (≥ 1400 s/mm², calculated or acquired)
ADC map from ≥ 2 b-values up to 1000 s/mm²
If any unchecked, score is automatically 0/4.
1.Adequate contrast and SNR on high b-value images
2.Adequate range of contrast to differentiate TZ/BPH from PZ on ADC maps
3.Absence of significant artifacts in the prostatic region
4.Anatomical matching of ADC / high b-value to axial T2-WI
DCE −
Dynamic contrast-enhanced
Essential prerequisites
Slice thickness 3 mm
Temporal resolution ≤ 15 s
Fat saturation (or post-processing: subtraction / heat maps)
If any unchecked, DCE is automatically ‘−’.
1.Absence of significant artifacts in the prostatic region AND appropriate bolus enhancement
2.Ability to identify anatomical structures (e.g. capsular vessels, pudendal artery)
Both criteria must pass for DCE ‘+’.

PI-RADS Lesions (v2.1)

up to 3 lesions · PZ dominant = DWI · TZ dominant = T2

Extraprostatic Extension

invasion of adjacent structures
The MRI EPE risk grade (Mehralivand 2019) is set per lesion on each PI-RADS 4–5 lesion above, and appears with that lesion in the report.
No lesions yet.

PI-QUAL guides whether to repeat a scan; withhold PI-RADS scoring for PI-QUAL 1 scans.
