O-RADS US v2022 and O-RADS MRI scoring for an ovarian or adnexal lesion. Answer a few taps, and only the questions that change the score appear. The MRI side includes a DCE time-intensity curve plotter that calls the curve type from signal values.

Score each lesion separately; management follows the highest-scoring lesion. When features conflict, use the one that gives the higher score.
Side
DCE time-intensity curve plotter
Enter solid-tissue signal-intensity values sampled at one fixed interval, first value = pre-contrast baseline. Add the outer myometrium series to separate intermediate from high risk. Without it, only a low-risk (no-plateau) curve can be called. The detected curve is a suggestion; confirm it against the plot.
No uterus, no reference. Plot the lesion alone and judge the shape, not the slope:
Place the ROI on the earliest-enhancing, most solid-looking part of the solid tissue, avoiding vessels and cyst fluid.
Record its signal on the pre-contrast phase and every dynamic phase, and enter those values above.
No shoulder (keeps climbing to the end): low-risk curve, O-RADS MRI 3.
Shoulder, then plateau or washout: intermediate and high risk cannot be separated without myometrium. Report it as at least O-RADS MRI 4.
Without DCE there is no way to compare at 30–40 s either, so a dynamic series is the one to ask for in these patients.

When does MRI help after an adnexal ultrasound?
Ultrasound is the first test for an adnexal lesion. MRI is a problem-solving test: it is more specific for cancer, so it mostly helps by showing that a worrying ultrasound lesion is benign. An adnexal lesion with no enhancement on MRI has a negative predictive value for cancer above 98%.
Ultrasound finding
What MRI adds
Lesion not fully seen (O-RADS US 0): large lesion, bowel gas, fibroid uterus
Shows the whole lesion and its origin; about 10% of lesions sent from ultrasound turn out not to be ovarian.
Non-simple unilocular cyst after menopause (O-RADS US 2)
Looks for solid tissue. No enhancement supports surveillance; enhancing solid tissue supports gyn-oncology referral.
O-RADS US 3 or 4
Often downgrades: in one US multicenter study, 68% of O-RADS US 3 and 48% of O-RADS US 4 lesions were physiologic findings or endometriomas. Enhancing solid tissue upgrades.
O-RADS US 5, when surgery would change
Young patients, fertility-sparing surgery, or a poor surgical candidate: 22% of O-RADS US 5 lesions in the same study were physiologic findings or endometriomas.
DHMC practice: after the diagnostic exam we refer to gynecology or gynecologic oncology, and that team orders MRI when it will change management.

1 Normal ovary
0%
None
2 Almost certainly benign
<1%
None, or follow-up US by cyst type and size (simple cyst >5 cm premenopausal or >3 cm postmenopausal: US in 12 months)
3 Low risk
1 to <10%
Gynecology; US within 6 months if not excised; MRI (O-RADS MRI) or a US specialist for solid lesions
4 Intermediate risk
10 to <50%
Gynecologic oncology involved; MRI or US specialist to characterize
5 High risk
≥50%
Refer to gynecologic oncology; imaging per their protocol
